Alaska DUI Records: Fax Request
Fax requests for the Alaska DUI records are also acceptable. All you need to do is download the Driving Record Request Form (Form 419F) available at http://doa.alaska.gov/dmv/forms/pdfs/faxdh.pdf and take a printout. You need to enter information in all the required fields and fax it to 1-907-269-5202. The processing fee for this procedure is to be paid via an approved credit card. The card details are mandatory in the driving record request by fax. The processing time for a fax request is approximately two business days.
In case you do not receive the driving records within this span of time you could send an email to DOA.DMV.WebMaster@Alaska.gov. The email must provide the full name of the requester, the fax number and the date on which the request was made. The Division of Motor Vehicles of Alaska would be able to check whether the request was received and inform you about the status of the request as well.
