Enclose a check of $17.75 for the first copy of the certificate and $10 for every additional copy. Except for the orders collected in person, all other delieverd requests require an additional fee as delivery charges.

To request a copy of marriage license via fax or mail, you will get the application form at http://www.cdphe.state.co.us/certs/marigver.pdf. The database of vital records holds the records of the marriages that had taken place in years 1900 to 1939, and from 1976 till date. The records of marriages that took place in the missing years are to be collected from the offices of respective County Courts.
For obtaining the application form of marriage dissolution verification, you can log in to http://www.cdphe.state.co.us/certs/mardis.pdf. You can receive the divorces that took place between 1851 and 1939 and again from 1968 to the present date. You will get the decree records for the dissolutions occurred between 1940 and 1967 in the office of County Court which had granted the decree. Fee for these records are $17. You will also be asked to submit documents to prove that you are eligible to request for these records.
