In Iowa, a DWI (driving while intoxicated) charge carries the same implications and consequences as an OWI (operating while under the influence) or a DUI (driving under the influence) charge. Fill it out and mail it to the Office of Driver Services at Des Moines. Attach a copy of your driver license or any non-driver identification document and a check of USD 5.50 drawn in favor of the Treasurer, State of Iowa.
